Dr. Sarah Frances Pugh was born in Lewis County, Missouri, to Civil War veteran Matthias Pugh and Hannah McWilliams Pugh. After completing her early education in Missouri, she taught instrumental music and worked as a nurse before pursuing osteopathic medicine at Dr. Ward’s School of Osteopathy in Kirksville, Missouri. She graduated from Dr. S. S. Still’s College of Osteopathy in Des Moines, Iowa, in 1902, completed specialized coursework in bacteriology and gynecology, and initially practiced in St. Louis.

In 1904, Dr. Pugh relocated her practice to Fresno, California, where she built a prominent osteopathic medical career across the San Joaquin Valley. She expanded her expertise through postgraduate studies in eye, ear, nose, and throat diagnosis in Kansas City in 1916 and Los Angeles in 1917, going on to publish several articles in her field. Beyond her medical practice, she served as Past Department President of the Daughters of Veterans of the Civil War for California and Nevada, held membership in the Order of the Eastern Star, and belonged to the Baptist Church.
